Do I need regular ultrasound?
How do I know if the endometriosis is returning after having surgery?
Please do not rely on ultrasounds to determine if you have endo. Endo does not show up on imaging. The ovarian cysts that develop and seen are not always connected to endometriosis. This is a way for physicians validate their lack of proper treatment and show their lack of knowledge on the disease.
Not every gyne is versed on it- they "know" what they were shown in school (very little). At the very LEAST make sure the surgeon is part of a pelvic pain specialty group (they do fibroids, adeno, endo and PID and others). The surgeon needs to excise- not just ablate.

@A MyEndometriosisTeam Member Im so sorry to hear that youll be needing a 3rd surgery so soon! My unsolicited advice is to please make sure the surgery is with a great specialist. I didnt, and deeply regret the decision.
Usually the obgyn will give you a yearly ultrasound but it can happen anytime you feel pain or heavy bleeding. I agree with @A MyEndometriosisTeam Member your body just kinda knows something is wrong and you also sometimes feel large lumps in your lower abdominal, your flow may get heavier and heavier, the pain is severely sharp and it feels like something is pulling at it, etc... I also am most likely going on my third surgery as well and it hasn't been a complete year yet. Best of luck to you.
After my 2nd surgery I could just "feel" that my endo was returning. (If that makes any sense). After a few ultrasounds they found a very large ovarian cyst. I had to go in for a 3rd surgery where they found my endo had returned only after a years time.
